TITLE:

Director of Nursing Services

STATUS:

Salaried, Exempt

PURPOSE:

To provide oversight for the day-to-day operations of the nursing departmentTo serve as a member of the Leadership Team of Lochland, Inc.

ACCOUNTABILITY:

The Director of Nursing Services is employed by Lochland, Inc. and is accountable to the Executive Director or his/her designee.

The Executive Director

JOB RESPONSIBILITIES, TASKS, AND PERFORMANCE RESULTS EXPECTED:

  • Function: Administration

Percentage of time spent: 60%

A. Tasks

  • The Director of Nursing Services will maintain personnel records for individuals s/he supervises to assist with evaluations.
  • The Director of Nursing Services will review all provider materials provided by the Medical Coordinator/LPN and update all necessary information in electronic and written charts.
  • The Director of Nursing Services will be responsible to ensure that all medications are entered in CareSuite in a timely manner and in a manner consistent with provider instructions.
  • The Director of Nursing Services will review all medication administration paperwork to determine if any medication errors have been made.
  • The Director of Nursing Services will ensure that Plans of Nursing Service are completed and distributed to all houses and programs in a timely manner.
  • The Director of Nursing Services will investigate all MOIRs that require a nursing review and ensure they are available for the Special Review Committee meetings.
  • The Director of Nursing Services will review all Head Injury Protocol documentation and ensure it is available for the Special Review Committee meetings.
  • The Director of Nursing Services will ensure that Medical Evaluations for the use of SCIP-R are completed yearly for every resident and for all new admissions.
  • The Director of Nursing Services ensure that semi-annual medication reviews are completed for each resident.
  • The Director of Nursing Services will ensure that medical chart audits are completed two times each year.
  • The Director of Nursing Services will ensure that all medical invoices are reviewed and approved and sent to the Finance Department in a timely manner.
  • The Director of Nursing Services will serve as the agency contact to the pharmacy(ies) providing medications to residents.
  • The Director of Nursing Services will provide quarterly reports to the Board of Directors.
  • The Director of Nursing Services will complete all POCA requirements in a timely manner.
  • The Director of Nursing Services will complete all IRC recommendations in a timely manner.
  • The Director of Nursing Services will complete any assigned time studies in the required time frames.
  • The Director of Nursing Services will attend all required training events.
  • The Director of Nursing Services will ensure that the nursing department is represented at all Life Plan meetings.
  • The Director of Nursing Services will ensure that the Nursing Department presents at each monthly residential All-Staff Meeting and Day Services Staff Meeting.
  • The Director of Nursing Services will provide an article for the Lochland Newsletter once each year, as assigned.

B. Performance meets expectations when:

  • The Director of Nursing Services has accurate personnel records for individuals s/he supervises.
  • The Director of Nursing Services reviews all provider materials provided by the Medical Coordinator/LPN and updates all necessary information in electronic and written charts within two (2) business days.
  • The Director of Nursing Services ensures that all medications are entered in CareSuite to ensure no missed dosages occur and in a manner consistent with provider instructions.
  • The Director of Nursing Services reviews all medication administration paperwork to determine if any medication errors have been made within a week of the administration and follows the Lochland Policy regarding Medication Errors.
  • The Director of Nursing Services ensures that Plans of Nursing Service are completed and distributed to all houses and programs within one (1) week of any changes or from the time of determining a new plan is necessary.
  • The Director of Nursing Services investigates all MOIRs that require a nursing review within three (3) business days and ensures they are available for the Special Review Committee meetings each month.
  • The Director of Nursing Services will review all Head Injury Protocol documentation within three (3) business days and ensure it is available for the Special Review Committee meetings each month.
  • The Director of Nursing Services ensures that all Medical Evaluations for the use of SCIP-R are completed by March 31stof each year for every resident and within one (1) month after the admission date for all new admissions.
  • The Director of Nursing Services ensures that semi-annual medication reviews are completed for each resident based on established deadlines.
  • The Director of Nursing Services ensures that medical chart audits are completed by June 30thand December 31steach year.
  • The Director of Nursing Services ensures that all medical invoices are reviewed and approved and sent to the Finance Department within one (1) week of receipt.
  • The Director of Nursing Services serves as the agency contact to the pharmacy(ies) providing medications to residents in a manner consistent with his/her supervisor’s expectations.
  • The Director of Nursing Services provides quarterly reports to the Board of Directors by the deadlines assigned.
  • The Director of Nursing Services completes all POCA requirements by the assigned due date.
  • The Director of Nursing Services completes all IRC recommendations by the assigned due date.
  • The Director of Nursing Services will complete any assigned time studies by the assigned due date.
  • The Director of Nursing Services attends all required training events and completes all online training by the dates assigned.
  • The Director of Nursing Services ensures that the nursing department is represented at all Life Plan meetings.
  • The Director of Nursing Services ensures that the Nursing Department presents at each monthly residential All-Staff Meeting and Day Services Staff Meeting.
  • The Director of Nursing Services provides an article for the Lochland Newsletter once each year, as assigned.
  • Function: Leadership and Training

Percentage of time spent: 20%

  • Tasks:
  • The Director of Nursing Services will attend all Leadership Meetings and be prepared to present all medication errors.
  • The Director of Nursing Services will serve, on a rotating basis with the Registered Nurse, as the RN On Call for the agency.
  • The Director of Nursing Services will serve on the Special Review Committee.
  • The Director of Nursing Services will serve on the Incident Review Committee.
  • The Director of Nursing Services will ensure that all initial and annual training for staff is completed in a timely manner.
  • The Director of Nursing Services will ensure that Direct Support Professionals are trained, as required by the Office for People with Developmental Disabilities, in medication administration and any additional tasks that may be delegated to them by a Registered Nurse.
  • The Director of Nursing Services will ensure that all medication errors are addressed in a manner consistent with the Lochland policy.
  • The Director of Nursing Services will maintain all trainings, certifications, and licenses as are required by Lochland, Inc., the Office for People with Developmental Disabilities, or the State of New York.
  • The Director of Nursing Services will complete other duties as requested by his/her supervisor.
  • Performance meets expectations when:
  • The Director of Nursing Services attends all Leadership meetings and presents all medication errors.
  • The Director of Nursing Services serves as the RN On Call for the agency in a manner consistent with agency expectations.
  • The Director of Nursing Services attends all meetings of the Special Review Committee.
  • The Director of Nursing Services attends all meetings of the Incident Review Committee.

  • Function: Supervision

Percentage of time spent: 10%

  • Tasks:
  • The Director of Nursing Services will provide daily supervision for the Registered Nurse and Medical Coordinator/LPN, including ensuring they have done any assigned tasks.
  • The Director of Nursing Services will hold weekly meetings with members of the nursing department and the Behavior Intervention Specialist, and keep notes on topics covered, issues addressed, and other relevant information.
  • The Director of Nursing Services will evaluate the Registered Nurse biannually based on his/her position description.
  • The Director of Nursing Services, with the Registered Nurse, will evaluate the Medical Coordinator/LPNs biannually based on his/her position description.
  • The Director of Nursing Services will provide progressive guidance for members of the nursing office staff, as required.
  • The Director of Nursing Services will ensure that all members of the nursing department maintain up-to-date training as required by Lochland, Inc. and OPWDD.

  • The Director of Nursing Services provides daily supervision for the Registered Nurse and Medical Coordinator/LPN, including ensuring they have done any assigned tasks.
  • The Director of Nursing Services holds weekly meetings with members of the nursing department and the Behavior Intervention Specialist, and keeps notes on topics covered, issues addressed, and other relevant information.
  • The Director of Nursing Services evaluates the Registered Nurse biannually based on his/her position description.

  • Function: Direct Care of Residents

Percentage of time spent: 10%

  • Tasks:
  • The Director of Nursing Services and the Registered Nurse will complete all individual consultations/assessments with residents related to medical concerns in a timely manner.
  • The Director of Nursing Services and the Registered Nurse will be responsible for initiating and discontinuing all Head Injury Protocols for residents.
  • The Director of Nursing Services will be responsible for assessing the ability of residents to attend programming events, as necessary, and to communicate that information to the appropriate parties.
  • The Director of Nursing Services will assist in the transportation of individuals to provider appointments, when necessary.

B. Performance meets expectations when:

  • The Director of Nursing Services and the Registered Nurse complete all individual consultations/assessments with residents related to medical concerns in a timely manner.
  • The Director of Nursing Services and the Registered Nurse initiate and discontinue all Head Injury Protocols for residents in a manner consistent with standards of care.
  • The Director of Nursing Services assesses the ability of residents to attend programming events, as necessary, and communicates that information to the appropriate parties.
  • The Director of Nursing Services assists in the transportation of individuals to provider appointments, when necessary.

EVALUATION:

In addition to ongoing feedback from his/her supervisor, there is a biannual evaluation of performance completed by the Executive Director and the Chief of Compliance and Strategy.

EMPLOYMENT TERMS:

This is a part-time position for thirty-two hours per week. Additional on call responsibilities are assigned on a rotating basis. The employee must always maintain confidentiality. Lochland, Inc. is an Equal Opportunity Employer. This is a salaried, exempt position.
