LPN

Lochland is looking for an LPN, OPWDD experience is preferred but not necessary.

  • The Medical Coordinator/LPN will coordinate the scheduling of all medical appointments for the residents assigned to him/her.
  • The Medical Coordinator/LPN will ensure that s/he has all information needed to communicate with the provider during any appointment for the residents assigned to him/her.
  • The Medical Coordinator/LPN will appropriately maintain all medical documentation in electronic or printed form for the residents assigned to him/her.
  • The Medical Coordinator/LPN will ensure that all medications and materials in the medication administration area of each home and the Day Habilitation Building are maintained appropriately and neatly for the houses/program assigned to him/her.
  • The Medical Coordinator/LPN will ensure that all prescriptions are stocked appropriately for the residents assigned to him/her.
  • The Medical Coordinator/LPN will coordinate the Weekly Appointment Schedule and ensure its distribution in a timely manner.
  • The Medical Coordinator/LPN will ensure that Administrative staff members (Executive Director, Team Leader, Residential Director, Behavior Intervention Specialist) are informed of any significant medical issues related to the residents assigned to him/her.
  • The Medical Coordinator/LPN will attend the weekly departmental meeting.
  • The Medical Coordinator/LPN will attend the monthly residential All-Staff meeting.
  • The Medical Coordinator/LPN will maintain all appropriate training requirements of the agency or other oversight agencies.
  • The Medical Coordinator/LPN will complete any other assigned tasks as requested by his/her supervisor.
  • Performance meets expectations when:
  • The Medical Coordinator/LPN completes the scheduling of all medical appointments for the residents assigned to him/her within two (2) business days of the request.
  • The Medical Coordinator/LPN completes any follow-up scheduling of medical appointments The Medical Coordinator/LPN will coordinate the scheduling of all medical appointments for the residents assigned to him/her at the appointment preceding any needed follow-up.
  • The Medical Coordinator/LPN will consistently demonstrate his/her preparation to communicate with the provider during any appointment for the residents assigned to him/her.
  • The Medical Coordinator/LPN will ensure that all medical documentation, in electronic or printed form, is filed or recorded within two (2) business days of receiving the paperwork for the residents assigned to him/her.
  • The Medical Coordinator/LPN will review, order, and maintain all medications and supplies in the medication administration area of each home and the Day Habilitation Building weeklyfor the houses/program assigned to him/her.
  • The Medical Coordinator/LPN will coordinate the Weekly Appointment Schedule and ensure its distribution in a timely manner no later than the Wednesday preceding the week being scheduled.
  • The Medical Coordinator/LPN will ensure that Administrative staff members (Executive Director, Team Leader, Residential Director, Behavior Intervention Specialist) are informed of any significant medical issues related to the residents assigned to him/her within an hour for emergent issues and within one (1) business day for non-emergent but serious issues.
  • The Medical Coordinator/LPN consistently attends the weekly departmental meeting and is prepared for and participates in that meeting.
  • The Medical Coordinator/LPN consistently attends the monthly residential All-Staff meeting and is prepared for and participates in that meeting.
  • The Medical Coordinator/LPN maintains all appropriate training requirements of the agency or other oversight agencies by assigned due dates.
  • The Medical Coordinator/LPN consistently completes any other assigned tasks as requested by his/her supervisor in a thorough and timely manner.
  • Function: Direct Care/Support of Residents

Percentage of time spent: 40%

  • Tasks:
  • The Medical Coordinator/LPN will be responsible for the transportation of residents assigned to him/her to all provider appointments.
  • The Medical Coordinator/LPN will ensure that another medical team member is available to assist with transportation of the residents assigned to him/her when necessary.
  • The Medical Coordinator/LPN will monitor and record the weights and blood pressure for the residents assigned to him/her on a monthly basis.
  • The Medical Coordinator/LPN will assist with direct care of residents in the houses or at the Day Habilitation Program, as needed.
  • The Medical Coordinator/LPN will maintain agency medical documents as assigned by RN.
  • The LPN will complete nursing tasks within the scope of his/her practice under the direction of an agency RN.
  • Performance meets expectations when:
  • The Medical Coordinator/LPN consistently transports residents assigned to him/her to all provider appointments in a safe and timely manner.
  • The Medical Coordinator/LPN consistently ensures that another medical team member is available to assist with transportation of the residents assigned to him/her when necessary.
  • The Medical Coordinator/LPN records and monitors the weights and blood pressure for the residents assigned to him/her no later than thefifteenthof every month.
  • The Medical Coordinator/LPN assists with direct care of residents in the houses or at the Day Habilitation Program when requested.
  • The LPN tasks are completed within scope of practice under the direction of agency RN.

EVALUATION:

In addition to ongoing feedback from supervisors, there is a biannual evaluation of performance, based on this position description, completed by the Registered Nurses.

EMPLOYMENT TERMS:

  • The Medical Coordinator(s) will have a minimum of a high school diploma and one (1) year of applicable work experience
  • Agency LPNs must have and maintain a current license in good standing with New York State Education Department
  • A valid NYS driver’s license
  • Current CPR/First Aid Certification within (60 days of hire.)
  • SCIP-R certification (Within 90 days of hire)
  • OPWDD Medication Administration Certification (Not required for LPNs at the discretion of Lochland RN)
  • Ability to work collegially with others
  • Ability to relate effectively to the residents of Lochland, Inc., its staff, Board of Directors, and other professionals in the field.

  • Professional appearance, ethical conduct, and ability to maintain confidentiality
